Site Policy for Out-of-Office Referrals

 In some cases, referrals for treatment with other professionals may be made when patient care exceeds the clinician’s scope of practice. These services may include, but are not limited to medical exams, medication assessment, management and/or prescription, psychiatric examination, psychological testing, social service benefits, pediatric assessment and well-child checks, inpatient treatment, hospitalization, or Emergency Department referral.

 Clinicians will:

  1.   Clarify services with the patient and upon agreement, get a Release of Information to the appropriate agency.
    2.     Assist the patient in accessing referred services (i.e. make phone calls, help set appointments, clarify costs, assure transportation).
    3.     Verify services available under this referral are offered to all without regard for the ability to pay. 
    4.     Follow up with the patient and the referred provider to establish treatment protocols, provide information and updates.
